REPLACE Entire set of Bid Forms with revised Bid Forms attached to and issued as part of this Addendum 1 Re-Bid.

CHANGE Article 5. DATES OF PERFORMANCE as follows:
This Service Agreement shall commence on date stated above and shall continue until June 30, 2019. This Service Agreement may be extended by mutual agreement upon 30 days’ written notice. Thirty (30) days before termination date, under same terms and conditions and upon mutual consent of Milwaukee County and Contractor this agreement may be extended for an additional term of one year until June 30, 2020. At the end of renewal term and under the same circumstances, an option to renew for a third year term may be exercised.
